おなか for hunger, fullness, and a hurting belly: when 胃 is the word you actually need
Use おなか naturally for the belly or abdominal area, hunger, fullness, and everyday discomfort without confusing the broad body-area word with the specific organ 胃.
お腹 is the ordinary, softer word for your belly or abdominal area. In everyday Japanese, it also anchors three extremely useful phrases: おなかがすいた for “I’m hungry,” おなかがいっぱい for “I’m full,” and おなかが痛い for “my stomach or abdomen hurts.” The word does not identify one organ, and pain there does not automatically mean that the stomach itself is ill.
The usual spelling is おなか or お腹. Dictionaries label the word as polite language and describe it as a softer way to say 腹. That does not make it childish or limited to one kind of speaker. It is a practical default when talking about hunger, fullness, or the body area in ordinary conversation.
Learn the three phrases as complete units. おなかがすく means to become hungry, so おなかがすいた is the casual past-state form and おなかがすきました is polite. おなかがいっぱい describes being full after eating. おなかが痛い reports pain somewhere in the abdominal area without naming a cause.
The nearby words have narrower or differently colored jobs. 腹 is the plain noun “belly” and appears in expressions such as 腹が減った. It can sound more direct or rough depending on the speaker and situation, so it is not always the best replacement for polite おなか. 胃 names the stomach as a specific digestive organ. 腹部 is the anatomical or clinical term “abdomen,” while 腹痛 is the compact label “abdominal pain” that can appear on a medical form. Bilingual medical intake forms list 胃が痛い and おなかが痛い separately, which is a useful reminder that they are not identical.
Hunger starts the lunch plan
Aoi and Shigeru are helping at a weekend neighborhood craft workshop. They have been setting up tables since morning, and by noon Aoi notices the familiar empty feeling. She says why she is ready for lunch, and Shigeru gives a natural response:
朝から体験教室の机を並べていたので、もうおなかがすきました。じゃあ、そろそろ昼ごはんにしませんか。おなかがすきました reports the current result of becoming hungry. In casual speech, Aoi could say おなかすいた, with が omitted, especially to someone close. Shigeru does not need a special response formula. 何か食べる？ or そろそろ昼ごはんにしようか turns the statement into a practical plan.
Fullness changes what happens next
They buy lunch at a food hall. Aoi cannot finish everything and decides what to do with the rest:
もうおなかがいっぱいです。残りは持ち帰ります。おなかがいっぱいです is about fullness, not simply the physical size of the belly. A natural reply can respond to the plan: じゃあ、持ち帰ろう or デザートはあとにしよう. The casual phrase もういっぱい can also work when the food context is obvious, but the full phrase is clearer for a learner.
A broad discomfort needs a broad word
After lunch, the pair walk back to the workshop. Aoi notices some discomfort but does not know which organ is involved:
ちょっとおなかが痛いです。少し休みます。Here おなかが痛い is the honest broad description. Translating it as “I have a stomachache” may be natural English, but the Japanese does not prove that the organ 胃 is the source. 胃が痛い is available when the speaker specifically means the stomach. If the speaker does not know, おなか avoids making that guess.
Shigeru can respond with 大丈夫？ and then ask for observable detail rather than suggesting a diagnosis. One useful question is どのへんが痛い？, “whereabouts does it hurt?” Aoi answers with the area she can identify:
どのへんが痛いですか。おなかの右側です。The structure おなかの右側 names a location without claiming a cause. Other practical location words include 上, 下, 左側, and 右側. In a medical setting, a professional may use 腹部, but おなか remains understandable everyday wording.
A second useful detail is time. Beginner textbooks and medical intake forms pair symptom descriptions with いつからですか, “since when?” Aoi answers only what she knows:
いつからですか。昼ごはんのあとからです。This does not explain why the discomfort began. It simply gives a timeline. The same communication pattern works with a form-like label such as 腹痛, but learners do not need to replace their ordinary sentence with clinical vocabulary to be understood.
The everyday decision is straightforward. For hunger, use おなかがすいた and respond by talking about food. For fullness, use おなかがいっぱい and respond to what the person wants to do next. For broad discomfort, use おなかが痛い; then a listener can ask どのへん？ or いつから？. The word stays broad while the rest of the conversation adds the useful detail.
